New Patient Forms
We encourage you to complete your medical forms prior to your visit to the clinic.
Before Your Visit
Please ensure you have filled out your health history. Also please bring a list of all medications and/or supplements you are currently taking. It is vital for our healthcare team to be able to review your medications and supplements. You may also bring any relevant laboratory results, X-rays, other imaging results, or clinical history relating to your diagnosis. You may get a copy of all records from your primary care doctor’s office or from the medical institution where the records originated.
During Your Visit
If you have never been to an integrative healthcare clinic before, you may not know what to expect. Fortunately, our staff are caring professionals who want to make sure your visit and experience is comfortable.
Your first visit includes a review of your medical history and present health concerns. If required, you may have to do lab tests as well as having a basic medical examination. Your exam may include: checking your ears, nose and throat, listening to your heart and lungs, and an abdominal exam.
During this period your doctor will help you select the best form of treatment that suits your needs and provide you with instructions on how to achieve your health goals. Our clinic staff will explain how to follow the advice as well as get the supplies you need.
While You Are Waiting
The waiting area has magazines and coloring books for your children. Parental supervision is required in the waiting area.
If You Are Running Late
Call us at +234(0)84304094 if you are running late for your appointment. If you are more than 15 minutes late, we may need to reschedule your visit.
After Your Visit
Should you have any questions, our clinic staff are available to answer your questions during normal business hours.
Return Visits
Your doctor will discuss with you the frequency and time frame for your follow-up appointment, after which the clinic staff will help you schedule your follow-up appointment so you have the preferred date and time that best suits your schedule.
